Change the Official Correspondent for a Registered Medical Device Facility

Use this option to reassign the official correspondent for any of the facilities under your account. From the Facility Selection screen select all facilities that will have the same correspondent.

Note: From this module you may only assign correspondents who have been assigned accounts. If you wish to assign a new correspondent who is not registered with the FDA, please visit the Accounts Management site.

Change Official Correspondent Main Screen

Facility Selection Screen

Select the facility you wish to change, and select the Change Official Correspondent option.

To complete the transaction, click the box next to the Certification Statement and Submit.

A confirmation will be displayed listing the facilities, their new official correspondent, and the previous correspondent(s).

To assign a different Official Correspondent to other facilities, click "Change the Official Correspondent for a Facility" from the left navigation bar.
